Job Summary
The Pool Nurse will deliver high-quality, compassionate care to patients at home, ensuring continuity of rehabilitation and recovery as directed by medical professionals.
Key Responsibilities
- Implement physician-directed care plans within residential environments
- Administer medications, wound dressing, and monitor patient vitals
- Educate families on hygiene, wound care, and rehabilitation practices
- Maintain patient documentation and compliance with safety protocols
- Adjust to flexible shifts and provide support across the patient care network
Skills & Competencies
- Clinical knowledge in rehabilitation and post-acute recovery care
- Proficiency in home healthcare standards and safety protocols
- Patient education with cultural sensitivity and empathy
- Effective communication with care teams and families
- Decision-making in emergency and adaptive settings
Qualifications
- Diploma or Bachelor’s in Nursing (valid PNC License)
- BLS/First Aid certification preferred
- Minimum 1–2 years’ experience in home care, med-surg, or community nursing
